Wait Times

All referrals are triaged and wait times vary depending on symptoms, but could be up to 12 to 18 months.

If you feel your patient's symptoms are urgent, a phone call to the on-call ENT would be recommended.

All referrals are triaged and scheduled accordingly, and must include the following:

Referring Physician Information
- First & Last Name
- OHIP billing number
- Phone number
- Fax number
Patient Demographics
- First & Last Name
- OHIP Health Card
- DOB
- Mailing or email address
- Phone number
Reason for referral
Patient's pertinent medical history
Patient's medication list
Previous ENT notes
Tests related to the reason of referral
- Imaging - CT, MRI, x-Ray, ultrasound
- audiogram
- DIZZY PATIENTS should have prior labs, EKG, and an MRI Brain while waiting for an appointment.
